The Board of Pardons clemency application cannot be obtained online and online versions from other sources will not be accepted.
At the present time, the only means of obtaining a Board of Pardons clemency application is by U.S. mail directly from the Board of Pardons Office.
To obtain an application, please send the following:
· Request for Clemency Application form
· $8.00 Application fee (non-refundable) made payable to the Commonwealth of Pennsylvania. Personal checks and cash are not accepted. Forms of accepted payment are:
- money order
- certified check
- cashier's check
- institution check
- law firm check
· Self-addressed business size envelope with $1.32 postage.
· Mail these items to the following address:

Upon receipt of the $8.00 Application fee, an Application for Clemency Packet will be sent to you.
